Membership

I, the undersigned, hereby acknowledge that participating in TMD  activities involves inherent risks, including but not limited to, physical injury, illness, or property damage. I voluntarily assume all risks associated with my participation or the participation of the minor listed above.

I hereby release, discharge, and covenant not to sue TMD, its instructors, employees, agents, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or the minor, or any property belonging to me or the minor, whether caused by the negligence of TMD or otherwise, while participating in any TMD  activity.

I understand and agree that this waiver and release of liability extends to all my heirs, executors, administrators, and assigns.

I have read and understand the terms of this membership form, including the waiver and release of liability. I agree to abide by the rules and regulations of [TMD  School Name].
